1. Our commitment

NADMedics and the independently licensed providers who care for you through our service are committed to protecting your protected health information ("PHI"): information that identifies you and relates to your health, care or payment for care. We are required by law to keep it private, to give you this notice and to follow it.

  • Health care operations. For quality review, provider oversight, training and running our service.
  • When the law requires it. Including public health reporting and reports of suspected abuse or neglect.
  • Serious threats. To prevent or reduce a serious, imminent threat to someone's health or safety.

3. Uses that need your permission

We need your written authorization for most marketing uses, any sale of PHI and most uses of psychotherapy notes. You can withdraw an authorization in writing at any time, except for actions already taken.

  • Paper copy. You can ask for a paper copy of this notice at any time.

5. Our responsibilities

We keep your PHI private and secure, will tell you promptly if a breach affects your information, and will only use or share it as this notice describes unless you tell us in writing that we can.

We can change this notice, and changes can apply to all information we hold. The current notice is always posted on this page with its effective date.
